Dear Local 802 Member,
UPDATE: NEW VACCINATION APPOINTMENTS HAVE OPENED UP!
We are thrilled that New York State has announced that all New Yorkers over the age of 16 are eligible for COVID-19 vaccines as of April 6.
We are working with the NYC Mayor's Office on an equitable vaccination program for members of Local 802 by appointment only. NEW VACCINATION APPOINTMENTS ARE AVAILABLE NOW.
Please read this e-mail carefully for instructions. The protocol below is based on our instructions from the Mayor's Office that have been used with other unions and organizations:
1. To participate, you must make sure that you're an active member of Local 802 by logging into the Local 802 membership portal at https://info.local802afm.org. Write down your Local 802 union card number; you'll need it below.
2. By applying for a vaccination through this program, you're allowing Local 802 to enter your personal information into a vaccination website that is administrated by the City of New York or its partners.
3. You hereby certify that you've never had a serious or life-threatening allergic reaction -- such as hives or difficulty breathing -- to a previous dose of COVID-19 vaccine or any component of the vaccine.
4. You hereby certify under penalty of law that you live in New York City or that you work in New York City.
5. The site will be offering the Pfizer vaccine.
6. The intention of this program is to give first vaccinations. After getting the vaccination at your assigned appointment, you will be given an appointment for a second vaccination.
WHAT TO DO NOW:
- Click here to schedule an appointment. The city is beginning this program by only releasing a limited number of appointments. If all appointments are full, please check the box on the form that says "Notify me when new appointments become available."
- You cannot send another person to take your appointment.
- Bring photo I.D. and write down your Local 802 union card number; you'll need it at the vaccination site. Wear a mask and follow all social distancing and safety instructions. The City of New York takes all responsibility for this vaccination program.
